Service Description
All services provided are delivered within the framework of the nursing process (assessment, nursing diagnosis, planning, implementation, & evaluation). Services provided encompass all practices defined by Louisiana Nurse Practice Acts and are delivered in accordance with the Standards of Professional Performance. Service options can be developed and/ or modified to meet your school’s health and wellness needs. The method used to customized nursing services for your school is described below along with a list of school health services required by the Louisiana Department of Education. MBSAI: Health & Safety Services will supply all tools and equipment required to perform services except to the extent that services must be performed on or with school equipment, as agreed to by school. Nursing interventions will be individualized based on students’ health needs as determined by the nursing process (assessment, nursing diagnosis, planning, implementation, & evaluation). • Assessment_ Nurse will use systematic dynamic method to collect and analyze data about students. Assessment includes, not only, physiological data, but also psychological, economic, socio- cultural, and spiritual as well. • Diagnosis_ Nurse’s clinical judgment about the student’s response to actual or potential health conditions or needs. • Planning_ Based on assessment and diagnosis, the nurse sets measurable and achievable short and long range goals for students. • Implementation_ Nursing care delivered according to care plan. Care is documented in the student’s medical record. • Evaluation_ Student’s status evaluated and care plan modified as needed. Without being exhaustive duties performed are as follows: • Establish a schedule to provide comprehensive services for age appropriate groups • Check immunization records on grades required by the State Department of Education and the Department of Health and Hospitals • Develop individualized healthcare plans • Review health and development history of all kindergarten and first grade students entering school for the first time • Conduct height and weight measurements, vision, hearing, scoliosis, dental and other screenings on students in designated grade levels (make referrals as needed) • Follow- up on all students with suspected health problems • Conduct conferences with teachers and/ or parents as needed • Serve as health resources person to school staff and as a liaison between parent, school, and medical home.
